Will I ever have more kids?

We're in our mid 20s and I have a child from my 1st marriage that will soon start Kindergarten. Right now I am on my Mom's medical insurance, but have to get off due to me getting married and not being able to use my former last name any longer. I am completely ineligible for medical insurance because I have rheumatoid arthritis. Not only am I against all forms of gov. aid, I am ineligible for it due to my income. Neither of my husband nor I's jobs provide medical insurance and even if my husband was to take out a private policy and get off his dad's insurance, I would not be eligible for his because of my RA and other pre existing conditions. We want nothing more than to have another child, but cannot. Should we look at home birth? My Mom is a nurse and put her foot down on that. What can be done?
